Accountant For Doctors

Accountant For Doctors

Your medical finances, handled properly.

You are likely earning from multiple sources — NHS salary, locum sessions, private practice, or a limited company — and the tax position is rarely straightforward. We work with doctors on exactly this: self assessment, IR35 reviews, NHS pension annual allowance charges, and private practice structuring. Fixed-fee pricing, no jargon, reply within one working day.

  • Self assessment filed accurately with all allowable expenses claimed
  • IR35 status reviewed so your locum structure holds up to scrutiny
  • NHS pension annual allowance charges identified before HMRC raises them
  • Private practice and limited company income structured tax-efficiently

No long-term contract. If things are not working after three months, you leave with clean records and nothing owed.

Sound familiar?

Multiple income streams, one very large tax bill.

Most doctors are not struggling with ignorance about tax — they are struggling with time. Between clinical commitments, on-call rotas, and the demands of private practice, sorting out self assessment, pension charges, and the right company structure tends to get pushed back until it becomes urgent. By which point the options are narrower and the bill is usually larger.

  • Self assessment filed late, or filed without all legitimate expenses claimed
  • Uncertainty over whether your locum arrangements fall inside or outside IR35
  • NHS pension annual allowance charge arriving with little warning and no plan

What on top of things looks like

Your tax position is visible well before any deadline. Compliance is handled, expenses are maximised, and the structural questions around private practice or locum contracting are answered before they become expensive problems.

  • Self assessment prepared accurately, with every allowable expense identified and claimed
  • IR35 status reviewed and documented so your locum structure is defensible
  • NHS pension annual allowance position modelled before the charge lands
  • Fixed monthly fee covering agreed services, no unexpected invoices

What you get

Services most doctors need from us

From annual self assessment through to IR35 reviews and private practice structuring, the scope is tailored to your situation and priced as a fixed fee.

01

Self Assessment And Personal Tax

Your SA100 return is prepared with all allowable professional expenses included — GMC fees, medical indemnity, CPD, equipment, and home working costs where applicable. You will know your tax liability well ahead of the January deadline, not the week before. HMRC correspondence is handled; you do not need to deal with it directly.

Included as standard
02

IR35 Review And Locum Structure

If you work through a personal service company or as a locum contractor, your IR35 status matters considerably. We review the working arrangements against the relevant tests and provide a documented opinion you can rely on. For those considering a limited company for private or locum work, we cover the structuring question clearly before any formation takes place.

Fixed-fee advisory
03

Tax Planning For Higher Earners

At consultant and GP earnings levels, the NHS pension annual allowance charge, the high-income child benefit tax charge, and the tapered annual allowance all interact in ways that can produce unexpected bills. We model these in advance so you can make informed decisions rather than reactive ones. Inheritance tax planning and capital gains tax on property are also within scope.

Why Supreme Consulting

Why doctors work with us over a generalist accountant

The difference between a generalist and a specialist is the number of questions you have to answer before getting a useful response.

🏥

No explaining your income structure

NHS salary, locum sessions, private practice receipts, and limited company dividends are a familiar combination here. You will not spend the first three conversations explaining how you are paid before getting to the question you actually asked. The specific challenges around annual allowance charges and IR35 for locums are understood before you mention them.

📅

Over twenty years of practice

Supreme Consulting has been working with owner-managed businesses and contractors since 2005. That length of practice means the advice is grounded in what actually happens — not what the textbook says should happen. For doctors with complex tax positions, that distinction is worth something.

💷

Fixed pricing, agreed in advance

The scope of your engagement is set out before work begins and the fee does not change unless the scope does. There are no hourly rates running in the background while you send an email. For doctors already managing unpredictable on-call schedules, at least the accountancy bill is predictable.

Getting started

Up and running in four steps

The process is designed to be as low-effort as possible on your side. Most doctors are onboarded with two conversations and a checklist.

1

A brief discovery call

We start with a no-obligation call to understand your current setup — income sources, existing structure, and what is or is not working. It takes around twenty minutes and you will leave with a clear sense of what we would cover and what it would cost.

2

A fixed-fee proposal

You receive a written proposal covering the services, the fee, and what is included. Nothing ambiguous. If the scope needs adjusting, that conversation happens before any work begins.

3

Onboarding and records migration

We handle the transition from your previous accountant, request the professional clearance, and bring your records onto cloud accounting if that is appropriate. You provide the information; we do the organising.

4

Ongoing, with no surprises

Deadlines are tracked, returns are filed on time, and planning conversations happen ahead of the relevant dates rather than after the bill has arrived. Your tax position is visible throughout the year, which is generally more useful than finding out in January.

Questions

Things doctors usually ask before starting

Do you understand the NHS pension annual allowance charge and how it affects higher-earning doctors?+

Yes. The tapered annual allowance and the resulting charge is one of the more consequential tax issues affecting consultants and senior GPs. We model the pension input against the available allowance and identify the charge before it becomes a self assessment surprise. If you are considering scheme pays elections or carry-forward calculations, those are within scope.

What does it cost and what is included?+

Pricing is fixed-fee and agreed upfront based on the services required. A straightforward self assessment with employment and NHS income would sit at a lower tier; a more complex engagement covering IR35, private practice accounts, and tax planning would be scoped and priced accordingly. The proposal sets out exactly what is included before any work starts and the fee does not change mid-year unless the scope does.

My books are behind and I am not sure my previous accountant did things correctly. Can you still help?+

Catch-up work is fairly common, particularly for doctors who have been managing self assessment themselves or whose accountant was not specialist in this area. We handle the professional clearance with the previous firm, review what has been filed, and identify anything that needs correcting or amending. The cost of catch-up is scoped and quoted before we begin.

Is there a long-term contract or minimum commitment?+

No long-term contract. Engagements run on a rolling basis with reasonable notice to end. If you decide to move on, your records are transferred in good order and there is nothing outstanding. The relationship works because the service is worth staying for, not because leaving is difficult.

I work as a locum through my own limited company. Does IR35 apply to me?+

It depends on the working arrangements, not the company structure. Locum doctors working in NHS settings fall under the public sector IR35 rules, which means the engaging body is responsible for the determination. Private sector locum arrangements are assessed differently. We review the specifics of your contracts and working practices and provide a documented assessment — not a generic answer.

When should I know what my tax bill is going to be?+

You should know well before the January payment on account deadline, ideally by October at the latest. We prepare self assessment returns from the moment the relevant information is available after April, which means you have several months to plan for the liability rather than a few weeks. For doctors with variable private practice income or pension charges, early visibility is particularly useful.

Ready when you are

Less to worry about, starting now.

A short call is enough to establish whether we are the right fit, what the scope would be, and what it would cost. No commitment required to have that conversation.
