Sound familiar?

Your tax return is more complicated than most people’s.

Doctors rarely have a single, tidy income stream. NHS pay, locum assignments through an agency, private fees, and possibly rental income can all land in the same return. Add the NHS pension annual allowance charge, potential IR35 questions if you operate through a limited company, and the picture gets cluttered quickly. Most accountants file what you give them and move on. That is not particularly useful.

  • Self Assessment left until January with multiple income sources still to reconcile
  • No one has flagged the NHS pension annual allowance charge until it arrives unexpectedly
  • IR35 status unclear for locum contracts and no one has reviewed the position properly

What sorted looks like for doctors

Your return filed well before the deadline, with every income stream accounted for correctly. The pension annual allowance checked proactively. Your IR35 position reviewed if it applies. One accountant who already knows how the numbers fit together.

  • Self Assessment completed in good time, across NHS, locum, and private income streams
  • Annual allowance position reviewed each year before HMRC raises a charge
  • IR35 reviewed for locum contracts, with a clear, documented conclusion
  • Fixed-fee pricing with no unexpected bills when something takes longer than expected

What you get

Tax services built around how doctors earn

Whether you are a salaried GP, a locum, or running a private practice through a limited company, the scope covers what you actually need.

01

Self Assessment and Personal Tax

Your SA100 prepared accurately and filed ahead of the deadline, covering NHS pay, locum income, private fees, dividends, and rental income in the same return. The pension annual allowance calculation is reviewed as part of the process, not treated as an afterthought. You know your tax liability months before HMRC sends the statement.

Included as standard
02

Limited Company and IR35 Advice

If you work through a personal service company or are considering one, the IR35 position needs to be properly assessed for each contract. We review the working arrangements, document the conclusion, and handle the year-end accounts and corporation tax return. Dividend strategy is considered alongside salary to keep your personal tax bill where it should be.

For locums and private practice
03

Tax Planning and Advisory

Doctors have access to useful reliefs that often go unused, from capital gains tax planning on property to inheritance tax considerations as income grows. We review your position annually and flag anything worth acting on before the tax year closes. The conversation is straightforward and the advice is specific to your circumstances.

Why Supreme Consulting

What makes this firm a reasonable choice for doctors

Two decades of practice means the questions doctors ask are not new ones. The answers tend to be quicker and more specific as a result.

🏥

Familiar with how doctors earn

NHS employment, locum assignments, private practice fees, and limited company dividends can all coexist in the same tax year. You will not need to explain the difference between a BMA contract and an agency agreement before getting a useful answer. The complexity is familiar; the treatment is straightforward.

📋

Pension annual allowance reviewed each year

The NHS pension annual allowance charge has caught out a large number of doctors who were not expecting it. We check the position as part of your annual review, not after HMRC writes to you. If a charge applies, you will know about it with enough time to plan around it.

💷

Fixed fees, no clock running

All work is priced on a fixed-fee basis before anything starts. You know what you are paying and what is covered. Questions during the year do not generate unexpected invoices, which is the way it should work when you are paying a professional to be available.

Getting started

Up and running in four straightforward steps

Most clients are fully onboarded within a couple of weeks. The process is designed to require as little from you as possible.

1

A brief discovery call

We start with a no-obligation conversation to understand your income setup, what your current accountant does or does not do, and what you actually need. It takes around 20 minutes and you will leave knowing whether we are a good fit.

2

A clear, fixed-fee proposal

You receive a written proposal covering exactly what is included, the annual timeline, and the fixed price. Nothing is left vague. If the scope changes, we discuss it before any additional work begins.

3

We handle the transition

We contact your previous accountant, request the handover files, and set up your records on the cloud. You do not need to chase anyone or organise paperwork between firms.

4

Your tax running properly

Your return filed on time, your pension position reviewed annually, your HMRC correspondence handled. Less to carry around in the back of your mind, which is frankly the point of having an accountant in the first place.

Questions

Things doctors usually ask before getting started

Do you understand NHS pension annual allowance charges and how they affect doctors?+

Yes. The annual allowance charge is one of the more common issues for doctors with growing NHS pension growth, and it is frequently missed until HMRC raises it. We check the position as part of the standard annual review for doctor clients. Where a charge applies, we help you understand the scheme pays election option and factor it into your overall tax planning.

What does it cost and what is included?+

Pricing is fixed and agreed before any work starts, based on your specific income setup and the services required. A straightforward Self Assessment for a salaried GP with locum income will cost less than a package covering a limited company, payroll, VAT, and personal tax. You will receive a written quote covering exactly what is included, with no ambiguity about what generates an additional charge.

My records are not in great shape. Is that a problem?+

It is a common situation, particularly for doctors who have been managing their own bookkeeping or whose previous accountant was not particularly proactive. We handle the catch-up work and will be straightforward with you about what is involved and the cost before starting. Most clients who arrive with records in some disorder leave considerably less worried about it.

Is there a long-term contract?+

No. We work on an annual engagement basis and you are free to leave at any point. If you do, we provide a full handover to your new accountant and return all your records. There is no financial penalty and no unnecessary process.

I work as a locum through an agency and also have some private clinic work. How does that affect my IR35 position?+

IR35 applies to work done through an intermediary, typically a limited company or personal service company. If you invoice your locum agency or private clinic through a company, the IR35 rules may apply to some or all of your contracts. We review the actual working arrangements for each contract individually and document a clear conclusion, which gives you a defensible position if HMRC ever asks.

Can you help me understand how much tax I am likely to owe before the Self Assessment deadline?+

Yes, and this is part of what we consider standard practice rather than an extra. Once we have your income figures, we prepare a tax estimate so you know what is coming. For doctors with variable locum income, knowing the number several months ahead is considerably more useful than finding out in January.
